Your Medicare benefits will remain the same

ACO REACH stands for an Accountable Care Organization (ACO) Realizing Equity, Access, and Community Health. This is a Medicare ACO model that aims to improve patient care.

Medicare has started an initiative where healthcare providers who share a common set of goals aimed at improving patient care can work together more effectively. This initiative brings together healthcare professionals in our ACO, to work together with Medicare to give you more coordinated care and services.

BENEFITS
If you were aligned with C3 by Medicare or have chosen to be aligned with our ACO REACH, your Medicare benefits will stay the same. You can continue seeing any Medicare healthcare provider you choose.

 

MEMBER ID CARDS
You will not receive an additional ID card and all services will be paid for by Medicare.

ACO REACH Governing Committee

Our Medicare ACO REACH Governing Committee and administrative leadership is composed of representatives from each health center participating in the program.

Member Alignment

To be eligible for our ACO REACH program, you must:

  • Be enrolled in Medicare Parts A and B

  • Have Medicare as the primary payer

  • Not be covered under Medicare Advantage, or other Medicare managed care plan

  • Be resident of the United States

  • Reside in a county that is included in our ACO’s service area

Selecting Your Provider and ACO

You may choose to join our ACO by designating a Participant Provider affiliated with C3 as your primary clinician or main source of care. It is your choice to choose the health care providers with whom you want to have a care relationship.  

When you choose a primary clinician that participates in our ACO REACH, they get access to tools and services to better coordinate your care and improve your health. Your Medicare benefits stay the same.
