WHO WE ARE

Community Care Cooperative (C3) is a 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs) in Massachusetts.

In 2016, leaders from 9 FQHCs throughout the state formed C3. An innovative model for an Accountable Care Organization (ACO), C3 leverages the proven best practices of ACOs throughout the country. C3 is the only ACO in the state founded and governed by FQHCs exclusively focused on advancing integrated and coordinated community-based care for MassHealth members.

C3 is building its organization on the collective strengths of health centers. We currently have 18 partner health centers across the state and our continued growth and expansion enables C3 to better serve MassHealth members across the Commonwealth. View a list of our health centers.

C3 believes in the power of community-based care; in preventing illness, addressing patient health conditions at the right time and place, and improving the overall wellness of MassHealth members through a deep understanding of their full life experience. "Great health is our primary purpose." 

C3 applies advanced data and population health management techniques to keep people healthy. C3 also offers MassHealth members access to the broadest network of hospitals and specialists. We understand the distinctive and diverse needs of our communities and are invested in their wellbeing and growth.

Most Accountable Care Organizations (ACOs) are led by large hospital systems or insurance companies.

C3 is different because the health centers that founded C3 are different. As the only ACO in the state founded and governed exclusively by FQHCs, we are uniquely positioned to effect an historic transformation in healthcare within the MassHealth network.

We understand that poverty contributes to illness. Health centers address social needs as part of our work to foster health and opportunity for the communities we serve. Health centers care for the whole person. Before integrated care became a buzzword, health centers were breaking down barriers between physical health and behavioral health.

In addition, National studies show the total cost of health care associated with health centers is 20% lower than other types of primary care, without any reduction in quality of care. These differences are just a few of the reasons we’re excited about our potential to change the world; one healthy and happy person at a time.
