  • Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve.

  • The Practice Transformation Manager will serve as the primary relationship manager to the FQHCs and Affiliated Participating Providers (APPs) providing oversight and assistance in achieving their annual cost and quality goals. The position will also support other required functions, such as regulatory requirements of C3 risk contracts.

This position requires flexibility and may vary from day-to-day to meet members where they are. Outreach methods may vary based on the needs of the organization and may include telephonic or in person in a variety of potential settings such as but not limited to, the *health center, community, home, or an inpatient facility. * This role is currently remote in accordance with Mass DOH Covid prevention guidelines. Upon lifted Covid restriction, it may require health center, medical/BH hospital facility, community, or homebased work.

  • The Certified Risk Coder will be a part of an emerging coding team and coding service that performs retrospective and prospective risk coding reviews and completes provider training for 18 outpatient primary care practices across Massachusetts. In so doing, he/she/they will use knowledge of appropriate coding, combined expertise in claims submission processes to improve the accuracy of documentation.

    This work ultimately leads to a greater understanding of the patient’s complexity while ensuring accurate risk adjustment for patient care. The Certified Risk Coder has experience in risk adjustment, outpatient primary care and/or behavioral health condition coding, billing compliance, and coding quality assurance protocols. He/she/they will report to the Manager, Risk Adjustment and interface with an internal team of Health Center Performance Managers, as well as staff at FQHCs.

  • The Application Analyst positions will be housed under a new subsidiary organization of C3, Community Technology Cooperative, LLC (CTC). To implement, maintain, support, and maximize the use of clinical, financial, and administrative portions of the software applications utilized by CTC in conducting daily operations under the direction of the Director of Applications. This includes:

    ▪ Maintaining current clinical, financial, and administrative applications, implementing new clinical, financial, and administrative applications

    ▪ Assisting in the assessment of workflow processes and assisting in process re-engineering to achieve efficiencies in departmental operations

    ▪ Assisting users in developing reports and analyses, troubleshooting, and resolution of application issues

    ▪ Working with the IT Education staff to assist in training users and in the development of documentation

    ▪ Maintaining a level of knowledge about IT operations and network issues and maintaining current industry knowledge

  • The Data and Reporting Analyst is a key member of the finance and analytics team working closely with the Director of Medical Economics and Performance Analytics, Director of Strategy & Finance, Chief Finance and Strategy Officer (CFO) and other members of the C3 team. The Analyst works collaboratively with all internal departments and with the Company’s FQHC partners to support the vision, mission and strategy of the organization. The Analyst must be able to analyze and draw accurate conclusions from medical claims and other forms of data; present and discuss findings in writing and verbally to a senior leader audience; and with limited direction complete tasks and larger projects. The Analyst must also have a customer-service mentality, recognizing the importance of both internal and external customers.

  • The Manager of PMO Services is responsible for managing priority projects based on strategic or operating importance to the organization and participating FQHCs and Affiliated Participating Providers (APPs) as applicable as well as oversight of the Project Management team. This will include projects to implement new products and services, optimize our model of care, improve business operations, and leverage the shared value of FQHCs and Affiliated Providers. In addition, the Manager of PMO Services will have project management responsibility for other initiatives and innovations at the organization and across participating FQHCs and APPs to meet overall organizational goals and provide additional support. This individual will work closely with the Senior Director of Operations, leadership, and executive team in prioritizing projects and reporting on progress.

  • The Pharmacy Technician is responsible for helping the Pharmacists perform daily tasks to serve customers. Their duties include filling and labeling prescriptions, interacting with customers to answer questions or give them their prescriptions and contacting customers to notify them when their prescription is ready.

  • The Community Health Center Retail Pharmacist is responsible for overseeing the distribution of medications to patients. Their duties include communicating with health center physicians to determine the type of medication and dosage for a patient, taking inventory of pharmaceutical drugs to reorder them based on local demand and interacting with pharmacy patients/customers to give them their prescriptions and answer medication questions. The Pharmacist will control medications by monitoring drug therapies; advising interventions. Additionally, completing pharmacy operational requirements by organizing and directing technicians’ workflow, verifying their preparation and labeling of pharmaceuticals, and verifying order entries, charges, and inspections.

  • The Project Manager is responsible for managing operational projects and duties for the Community Pharmacy Cooperative (CPC), a subsidiary organization of C3, and for our participating FQHCs and Affiliated Participating Providers (APPs). This will include tasks that support the Manager of PMO Services, Operations, and Pharmacy services in gathering resources, information, and equipment to implement upcoming projects. Main duties include creating comprehensive plans, managing project plans supporting Operations, Pharmacy, and other projects with operational and compliance significance, maintaining work schedules, organizing and participating in stakeholder meetings, performing certain administrative functions and activities, and ensuring all deadlines for certain projects are met.

  • The Clinical Pharmacist position will be housed under a subsidiary organization of C3, Community Pharmacy Cooperative, LLC (CPC). CPC is focused on meeting the pharmacy related needs of the ACO and other cooperative members.

    This position is a unique opportunity for a Clinical Pharmacist to provide medication management expertise in an ACO environment. Through direct patient care, the Clinical Pharmacist will leverage their clinical expertise and knowledge of medications to ensure our patients have better health outcomes. The Clinical Pharmacist will work across departments to support Clinical Pharmacy Services as they relate to quality management, care management, and practice transformation. The Clinical Pharmacist position currently operates in a hybrid work model with many responsibilities achievable remotely.

  • Reporting to the Chief Business Officer, the Senior Director of National Strategy is responsible for the overall planning, organizing, and execution of C3’s national strategy, which includes: national recruitment of FQHCs into C3’s Medicare risk contracts; development of Medicaid risk partnership opportunities outside of Massachusetts; national policy and advocacy that supports C3’s mission and strategic aims; and developing a national business for C3’s service offerings (consulting, etc.).

    The Senior Director provides leadership and day-to-day management to the National Strategy Team and oversees a budget and associated vendor partnerships for marketing, public relations, advocacy, business development, and related travel expenses. The Senior Director is a matrix leader who will pursue these responsibilities in close coordination with peers and colleagues throughout the organization.
