Community Care Cooperative (C3) is an Accountable Care Organization (ACO) created by Federally Qualified Health Centers (FQHCs) to better serve their communities. Our health centers are committed to providing more opportunities for individuals to receive coordinated, holistic, and culturally appropriate care in the communities where they live and work.

So what does this mean for members?

With C3, your MassHealth benefits remain the same and you may see any specialist or hospital that accepts MassHealth or Massachusetts Behavioral Health Partnership (MBHP) without any new paperwork or rules. If you already receive care at one of C3’s participating health centers, you keep your health center PCP.

Ready to Enroll in C3?

Experience the C3 difference — get the best of community-based care while accessing the broadest network of hospitals and specialists. 

To be eligible for C3, you must…

  • Have MassHealth Standard, CarePlus, CommonHealth, or Family Assistance plan types

  • Be younger than 65 years of age

  • Have no other health insurance

  • Reside in the community (not in a nursing facility or chronic inpatient hospital)
